Excellent question! Unfortunately your nipples are high and the implants are low. Good news is that your breast fold is not the problem. The problem is that there is too mush space between your nipples and your breast fold so your implants, and the bottom part of your breasts are falling over the breast fold. This is called pseudo ptosis and is an easy fix!!! I believe pocket revision with lower breast pole reduction/lift is your best option. This will reposition your implants and will reduce the amount of skin and tissue between your nipple and you breast fold. Giving you tighter lifted breasts with a more aesthetic nipple positioning. I would recommend a revisionary surgery to set the appropriate distance from the nipple to your breast fold. This is a common problem that may happen if there was no horizontal removal of excess skin and breast tissue in the breast fold/lower pole of the breast. You may consider a revision to reposition the breast implants. The distance from your nipple to your breast crease appears to have stretched or been designed too long with the original operation. You need a revision lift to correct the lower breast skin excess.

Your nipples aren’t too high. Your implants are too low. It looks as if you had a lollipop type lift but it left the lower skin too loose. It should be converted to a full lift which will tighten the lower portion of the breasts so the implants will sit in the correct place

Hello!Nipple position looks like it might be fine, but they are giving the impression of looking low since the fullness provided by your implants is low. Setting the implants higher will better center your nipples. You should circle back to your plastic surgeon to get measured and discuss your options. Shortening the lower pole of the breast and possibly utilizing a support scaffold like Galaflex may provide you a more pleasing outcome.Best of luck to you!
Your implants are too low. In order to get nipple/areola to be on top of breast it would be necessarry to lift inframammary folds and move implants higher. You should discussed this issue with your surgron and/or seek a second opinion consultation in person. Good luck.
You might try a round of oral Accolate and external ultrasound treatments to see if the capsule can be softened and the implant settle down. You may also need to wear the band.
You will probably need the vertical lift which will do a better job with shaping the breasts. Don't go too large with your implant, or you will sag again.
You probably had large areolae. these can be trimmed and tightened in the office under local anesthesia if you want. Your large nipples can be trimmed down as well if desired.
