Not many patients are good candidate for a peri-areolar mastopexy as this technique ONLY tends to work to reduce the size of the areola with minimal actual lift. In addition, peri-areolar scars tend to be wide, which can be particularly problematic in individuals with more pigmented skin. Please proceed with caution and make sure you see a very experienced board-eligible or board-certified plastic surgeon trained in breast surgery.
It depends on how much of an improvement you’d like. A donut lift achieves only a minimal improvement so it may not give you the result you want.
The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Based on these photos, you need much mre lift than donut breast lift may offer. Vertical breast lift with lollipop scar would be my choice. Good luck.
Great question--your breasts do not appear to be ptotic, as your nipple-areola complex is above the level of the fold underneath your breast. For a fuller, perkier breast, you would do well with just a breast augmentation, either with implants or fat graft (or both) depending on your size and shape goals. Peri-areolar mastopexies have a very limited utility, as they tend to stretch out the areola and look unnatural with time.
I appreciate your concerns, and it appears from your photos that a modest lift can be achieved with the Peri-areolar approach. However, a much more effective lift will be achieved if a vertical component is added. I understand that this results in more scarring, however, that technique allows your surgeon to lift and shape your breasts in a better fashion. Dear Inventive962298,my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. You will probably need the vertical lift which will do a better job with shaping the breasts. Don't go too large with your implant, or you will sag again.
You have a slight fall that does not allow you to only make it areolar. that is, you have to make a small scar under the breast. but it is minimal in your case. If you only do it areolar you will have an implant that will fall out sooner than normal. I do not recommend it to you. get a little lift
You probably had large areolae. these can be trimmed and tightened in the office under local anesthesia if you want. Your large nipples can be trimmed down as well if desired.
You may need liposuction with minimally invasive radiofrequency skin tightening like Renuvion for your abdomen. The crescent lift is not a good choice and seldom works well. You might qualify for the doughnut lift, but will have better shaping of your breasts with the vertical lift. The doughnut...
Your photo is taken with your arms raised so it does distort and raise your breast some. Measurements are needed to be certain. If your breasts look the same with your arms down by your sides, then you might do well with just an augmentation, especially if you want the more "natural " look.
