Hello, 34 years old mom. Breastfed for two years & looking into getting breast fat transfer (might need a lift) as well as abdomen tightening. I would like to avoid implants and major tummy tuck surgery. Is that possible? I don’t have any wish pictures as I love my body the way I am but would like to correct post pregnancy changes.

Answer: Possible, but would temper expectations Breast fat transfer is an excellent procedure that will harvest fat from the abdomen or other areas and transfer the fat to the breast. The effect that the fat transfer will have is highly dependent on the amount of fat there is to transfer. The fat is typically harvested from the abdomen, flanks, back, and inner thighs. Based on these pictures, it would seem as if you would have minimal fat to harvest. This, overall, is a good thing as it means that you are overall of a healthy weight. However, it means that you may require a different plan to give you the results you desire. Excess fullness in the abdomen is likely due to skin excess and not fat excess, which would require a mini-abdominoplasty (tummy tuck) or full tummy tuck. Breast rejuvenation may best be achieved through a lift with or without implants. Only a full physical exam with a board-certified plastic surgeon will be able to give you the appropriate full surgical plan.

You would need a lift to create more perky breasts. The abdominal tightening is a tricker question. It does appear that you have loose skin as evident from the wrinkling of the lower abdominal skin from your photos. It is hard to appreciate if there are stretch marks. You also have loose appearing muscles in the lower abdomen because there is bulging there. While liposuction will cause some skin tightening, it concurrently creates excess skin by removing the underlying fat. It is hard to determine without an exam whether your skin will recoil after liposuction. Liposuction would not address the muscles. I would recommend consultation with a board certified plastic surgeon to discuss your concerns.
