Breast augmentation bottoming out or just dropped and fluffed? (Photo)

The angle of your photos makes it difficult to definitively answer your question, but the photos suggest your implants may be sitting low in relationship to the nipples and breast mounds.

It appears that you have "bottomed out" likely due to poor incision placement, poor technique, inappropriate implant selection/size/profile, and natural causes from weak tissues at the inframammary fold. You may need some internal support as well such as Galaflex. I would find a surgeon certified by the American Board of Plastic Surgery; ideally, one with an aesthetic fellowship accredited by The Aesthetic Society. I would also review before/after photos, Google reviews, and talk to the surgeon directly if possible.

I think your implants have bottomed out a little. This can be fixed with suture techniques and even the potential to use Galaflex or other type of support scaffolding. Talk to your surgeon about these possibilities. Best of luck!

Thank you for your question. A large portion of my practice is dedicated to breast surgery and revision breast surgery. I have been in practice for over 28 years. In your case, you would benefit from an internal bra procedure - capsulorrhaphy. I wrote the very first paper on capsulorrhaphy over 20 years ago. I find that many surgeons are not skilled at this procedure and have high rate of recurrence. You should be able to have an excellent result. Best wishes to you.

Yes, your implants are bottoming out, the nipple-areola complexes are going higher and the implants are going lower. The only thing holding up your implants is the skin envelope. You will need a revision done to have the pockets reinforced inferior and laterally to prevent further descent.

It's hard to tell from the photos if that is just normal settling or if it's minor bottoming out. If you look straight on (your photos are looking from underneath), are your nipples basically in the center of your implant, or are they near the upper third of your implant? If they are near the top of your implant, see your surgeon and discuss.

Unfortunately, it appears you've bottomed out. All your symptoms are normal and not associated with the drop out. The reality is that capsular contracture has become much less of an issue as surgeons around the world recognize that avoiding bacterial biofilm contamination of the implant as an important aspect of the surgery. The flip side of the coin is that there is now a greater tendency to see less of a foreign body reaction to the implant, leading to insufficiently strong capsule formation and implant migration. Here in the States where we almost exclusively use smooth implants (textured devices carry a small risk of a unique type of a white cell cancer known as lymphoma), which exacerbates this problem. It looks like your surgeon nailed your incisions down nicely too, but despite that you dropped out. You will need a revision surgery to tighten the capsule and reinforce that repair with absorbable mesh. I'm sure your surgeon is disappointed too.

You are actually bottoming out. I would suggest wearing a good supporting bra at all times except in bed. Please discuss this with your surgeon.

Looks like the implant dropped below the fold. I would repair with Galaflex mesh for support. You can keep same implants if happy with size
