I recommend a circumareola approach. Excess skin can be removed from medial around the areola to move the nipple-areola complex more medial and the pocket can be dissected more medial and the lateral pocket can be reinforced as well. At the same time, a lower profile implant can be placed.

I'd suggest a lower profile implant with a wider base diameter. Explant the pocket medially and in the superior pole and use suture to close off the pocket laterally. You can reinforce the pocket repair with mesh/internal bra such as Durasorb or Galaflex.

Thank you for your question. A large portion of my practice is dedicated to breast surgery and revision breast surgery. I have been in practice for over 28 years. In your case, you would benefit from an internal bra procedure - capsulorrhaphy. I wrote the very first paper on capsulorrhaphy over 20 years ago. I find that many surgeons are not skilled at this procedure and have high rate of recurrence. You should be able to have an excellent result. Best wishes to you.

With your anatomy, you will always have a wide gap. The bases of your breasts cannot be moved. However, the lateral pockets can be tightened to decrease movement some to the sides. You could consider going down in profile with a wider implant. You don't appear to have donor fat, so fat transfers are not an option.

Several things can be done to improve the look of your breasts.
1 Place implants above the muscle
2 Mesh support
3 Moderate profile implant
4 Fat grafting

High profile breast implants may not have been the best choice for you. In general, they have a narrower base then moderate or moderate plus breast implants. Before photos would be helpful to determine if wider based implants would have lateralized (pushed apart) your nipples more. Return to your surgeon for a discussion of your options. Good luck!

There are three variables that determine the quality of breast documentation surgery.
The first valuable is the patient candidate for the procedure.
This includes valuables, like baseline a symmetry, breast position on the chest wall and breast divergence.
The second variable is the choice of implant in regards to type size and shape.
The third variable is the technical ability for the surgeon to place the implants in the anatomically correct position.
Surgeons cannot move your breast on the chest wall.
Each implant needs to be centered to each breast.
The ideal placement of an implant is so that there is an equal distribution of breast tissue on all sides . In other words, the implant should be placed where the breast is not where the patient wants the breast to be.
One option to create more fullness in the media aspect is to use survivor diameter breast. This would mean a lower profile implant.
Individuals who have divergent breast or breast that sit wide on the chest wall need to recognize that this will be amplified with the projection of the implant.
It looks like you implants replaced centrally under each breast in the correct anatomic position.
It sounds like your surgeon told you about this before the operation.
I suppose, in a perfect world, you’d be showing pictures of previous patients, who also had wide set breasts with divergence so you would have a better understanding of what your results would look like.
Surgeons cannot move your breast position on your chest wall. This is the way you were born and placing the implant where your breast is not isn’t going to fix the problem.
If you’re not content with the carrier receiving from your current provider, then I suggest scheduling a few in person second opinion, consultations with other plastic surgeons in your community.
For second opinion consultations come prepared bringing with you a complete set up before and after pictures and copies of your operative report.
These should be available from your current provider if you ask for them.
