Hello! I was wondering the liklihood of getting this extra skin smoothed out.l I was not interested in a FDL, So now I am trying to manage expectations. I understand I have some flared ribs, But I’m referring to the skin that’s under underneath them

Hello, thank you for your thoughtful question &mdash; you’re asking the right things.Based on what you’re describing, the excess skin you’re noticing beneath the ribcage is typically related to horizontal AND vertical skin laxity, not just localized looseness. While flared ribs can accentuate this area, the issue you’re referring to is usually skin that has lost its ability to retract, especially after weight changes or pregnancy.If avoiding a fleur-de-lis is a priority, it’s important to understand that no non-FDL option can reliably smooth all of the skin you’re describing. Thank you for the question. Based on your photos, an extended tummy tuck or body lift without FDL should be adequate to remove most of the excess skin.

Your photos are somewhat limited. It appears with your arms elevated that your breasts are still hanging low on the chest wall and you have extra axillary skin as well. There is a procedure called an upside down tummy tuck where the tummy tuck incision is done underneath the breasts to lift the upper abdomen. There is another alternative for only a breast lift called The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. This technique also lift the upper abdomen. At the same time, a panniculectomy or tummy tuck can be done.

Unfortunately, after massive weight loss, more than one operation is needed for difficult body areas.

Traditional tummy tuck removes lower skin in a vertical direction.

Since it doesn’t always address horizontal width, in my experience, fleur de lis will remove most of the excess skin under ribs and around torso best. Some liposuction can also be added.

Trade off is vertical scar all way up and some residual thickness at upper ribs, but excellent results are usually achieved. Bear in mind, FDL is not simply a T bridal extension, but dramatically will reduce and contour waist, torso. As such, not all plastic surgeons have performed many of these.

An exam and consultation with a plastic surgeon who performs these procedures is recommended to confirm you are a candidate as well as discuss your options and expectations.
