Question

What are the odds of getting the sides smoothed out with an extended tummy tuck without a FDL or reverse tummy tuck? I want to have realistic expectations. Also getting a breast lift and augmentation and arm lift.

What stands out in the photos is laxity and folding high on the upper abdomen/lower chest area, with some lateral fullness. An extended tummy tuck can help the lower abdomen, hip roll, and flank skin when the looseness is mostly horizontal and low, but it has limited reach for skin that sits directly under the breasts.
If the main excess is vertical midline laxity, an FDL addresses it better; if it is high upper-abdominal skin, a reverse tummy tuck may be more direct. Liposuction can help only if the problem is fat with reasonable skin recoil. Because you are also considering breast surgery and an arm lift, an in-person plan should prioritize scar placement, blood supply, and whether staging is safer.

Much of the skin looseness on your abdomen would probably be nicely improved with a tummy tuck.  You could probably suspect little or no improvement of the fold of tissue on your right flank, or side.  This may be acceptable to you or not, but an in person consultation with a plastic surgeon could give you much more information about what to expect.

The loose skin under the breasts can be helped by the Bellesoma. This technique reshapes your breast tissue creating upper pole fullness without implants, elevates them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. This technique will decrease the probability of needing a fleur de lis because the upper abdominal skin will be pulled up. This can also include the arms by doing a brachioplasty as well as axillaplasty to take care of the hanging skin.

CallieDreaminnn,It will be difficult to redrape the excess upper abdominal skin fold with a traditional abdominoplasty alone. Extended the incisions to your flank or even 360 degrees may not allow for the smooth contour you desire. The fleur de lis abdominoplasty will pull in horizontally along your upper abdomen, but your laxity is also vertical. A reverse tummy tuck does help that area, but will not help with your significant lower abdominal laxity. If you stage all your procedures, your breast lift could incorporate elements of a reverse tummy tuck to improve that area. Dr Stein

You can anticipate improvement in the abdominal contour with an extended tummy tuck but may still have some loose skin, which may be more noticeable in certain positions.  To get the best outcome a fleur de lis approach would be recommended for you.  This would remove skin from both the horizontal and vertical planes for the maximum improved contour.
