@rhiannajl Thank you for your question! Based on what you are describing I would recommend you discuss this with either your Plastic Surgeon or GYN. Did you have any pain/lumps in your breast prior to the implants or if this is new after surgery? 
It is likely that they will recommend breast imaging such as an US to evaluate the lump you feel

I get quite a sore/tender swollen lump in my left side, it’s to the left of my implant. This pain only comes before menstruation starts and it lasts for about a week. I understand that a lot of girls get breast pain around this time, but mine is incrredibly bad and only localised to one spot inside my breast. I have had my implants for over 2 years now, they seem fine and I dont think I habe any issues specifically with the implants, I’m just concerned about the lump and what it may be. Its quite large too, about the size of a grape

@littereatinglife Thank you for the question and including photos! This really helps me evaluate your body and provide specific recommendations. 
Based on your photos I would recommend liposuction and a traditional long scar (from elbow to armpit) arm lift to address your concerns. This will address the fat tissue/lipedema and allow your surgeon to remove the excess skin, which will be more significant following liposuction. I honestly believe you will be disappointed, if you proceed with lipo and a skin tightening (Renuvion or others).

I'm a 41yo with diagnosed lipoedema in my upper limbs. My BMI is currently 30, but I am aiming to lost another 10kg before any procedures. I need to have liposuction to remove the lipoedema fat, and I'm wondering if lipo plus Renuvion skin tightening would give a good enough result, or if I definitely will need an arm lift with the lipo. There is obviously a big cost, recovery and scar difference between the two options. I don't want to go for the more severe option if its not neccessary, but I also don't want to go with the less invasive option and be disappointed with the results. I mostly want to be able to wear tshirts with the arm not being tight and wear sleevless tops without being embarrassed.

@Calm349680 This is a great question. It is normal to have some loose/excess skin at the fold over the elbow (also known as the AC fossa). The reason this skin exists is because you need to be able to straighten your arm and that extra skin allows you to do that. 
If you still have loose skin when your arm is completely straight I would recommend discussing this with a Plastic Surgeon in your area. You may be able to have skin tightening procedures or skin removal. However, please be advised that this is not traditionally addressed with a standard arm lift.

CO2 lasers work by resurfacing the skin and stimulating collagen production. The long term benefit is pretty significant but it is hard to give an exact amount of time that each patient will have "good results last" because this depends on your skins starting point, how much benefit you actually get and how you protect your results. 
My recommendation is to be aggressive with sun protect and ensure you have a good skin care routine following the procedure to optimize the benefits you will see.

I am a 73 Year Old Male -— and scheduled to get a C02 Facial Laser next week. I have mild wrinkles, acne scars, and age spots Once I have Fully Healed from the procedure and seeing expected results - How Long do the “good results” last - how many years ?

@fjbdhh Thank you for your question and including photos. Liposuction over the back (including buffalo hump) will help to remove some excess fat in those regions but it will result in additional loose skin and won't change your underlying bone structure. 
What I mean by this is that the excess skin you already have in the upper back will likely get worse, the best way to address this would be with a torsoplasty (bra-line lift). Your bone structure (shoulders) is what gives the triangle shape and this will not be addressed by liposuction.

Typically with saline breast implants if there is a leak your body will absorb the fluid and that breast will "go flat". Depending on how large the defect in the implant is will determine how quickly that fluid will leak. This process is NOT dangerous to your body because the fluid used to fill the implant is sterile, even if it was placed 29 years ago. 
With all of that said, I would recommend you be seen by a Plastic Surgeon if you are experiencing new pain/changes in your implant.

I have had saline breast implants for 29 years. I went to get them checked out about 4 years ago, and the doctor I saw said I shouldn't do anything because they looked good. Well, now I am having an aching feeling in my left boob. It's a little sore. The implant feels a little less firm, but I know with Saline, typically, you know when it has leaked. Can Saline implants have a slow leak? Is it dangerous with the age of the breast implants?

@Chrcharity it is very high risk to perform a TT in the setting of prior unprovoked PE. Prior to considering surgery, you need to undergo a hypercoagulable work up to ensure there is no underlying disorder that caused you to have the PE in 2024. If that is negative, it would be reasonable to proceed with surgery assuming your plastic surgeon and hematologist have a plan for anticoagulation before/after surgery and you understand the risk of another DVT/PE from elective surgery.

I'm a 39 year-old Afticrican American woman and of 4 kids: Ages 10, 8, 5 and 3 years old. I had an unprovoked PE in June 2024 and have been on Eliquis since then, likely for life. Otherwise, I’m healthy and interested in getting a tummy tuck and liposuction. Both my hematologist and primary care doctor have cleared me for surgery. My hematologist’s plan is to stop Eliquis 48 hours before the procedure and restart it 24 hours after. I’ve consulted with three board-certified surgeons: -Two said they were not comfortable performing the surgery in their outpatient surgery centers due to my history. They recommended having it done in a hospital, but I’ve had trouble finding a surgeon who operates in that setting. -One board certified plastic surgeon said it wasn't worth the risk for them and essentially told me to be happy I'm alive after a PE and she couldn't live with something happening to me especially since I have 4 kids. -One surgeon said they would proceed in their surgery center. They seemed unbothered my PE and said they are already aggressive with clot prevention (heparin before surgery and Xarelto for all patients 24 hours after). She uses propofol for anesthesia. -Another surgeon also seemed fine with proceeding with the procedure in his surgery center but said he would want me to have an overnight stay there with his nurse. He recognized the risk but said that he would follow the protocol my hematologist suggested and wouldn't want to flip me to do any lipo on th

@Cool816775 Based on your photo it seems more like loose skin than residual fat (especially over the flanks), this could be confirmed by physical exam. 
If it is extra skin, it will not be addressed by additional weight loss or exercise.

Hey, I am currently 155 lbs 6'0 and M24. I used to be 220-230 and a little shorter back when I was 16-18ish and have lost the weight since then, i floated around 180-190 for a bit but now have been solid at this weight for a while. I recently 3 weeks ago just started focusing on lifting x3 a week while eating 1900 cals with 150g protein daily. I plan on atleast seeing what I can do change wise in the next 3-6 months before making a decision but I feel so stuck currently, its almost like a "catfish" feeling cause i look so lean in clothes but im still stuck with my loves handles and lower stomach softness which I cant tell if its mostly leftover fat or looseness. What do you guys think? Thanks!

@Aimsee This is a great question! It is possible to go down in implant size but there maybe some adjustments that need to be made to the soft tissue of the breast (such as capsule work or a breast lift). 
Depending on the implant selected the current breast capsule will likely need to be tighten or partially removed (These procedures are called capsulorrhaphy & capsulectomy). Additionally, depending on your current anatomy and tissue quality some patients need a breast lift when they down size implants. This allows us to address the excess skin that was created by the larger implant. 
I recommend discussing your concerns of the implants being too wide with your surgeon and determining what options exist.

Hi! I am going to have my current implants replaced soon. They are 9 years old. I’d like to go down in size. My implants are too large for my frame. One of the things that I don’t like about my implants is how wide they are. I was told by one doctor that I could not go more than 1cm down in width otherwise it would create too much room which could lead to fluid and issues. I’m confused because I know people have breast reductions all the time. Is it just too complicated to revise the capsule area and this is his hesitation?? how do you typically handle this? Il looking to go down from 485 cc to around 490 cc. But if I keep basically the same width I feel like they will just look still too wide for my body and then not as full. So wide and flatter in a way rather than smaller overall. For reference my first set of implants were smaller and I’m looking to go back to that size so I know my body can handle the smaller size.

Reconstructing the capsule - is this too complicated?

Talk with your surgeon about your concerns. It would be best to start acyclovir or valacyclovir prior to laser resurfacing to avoid it causing a flare. These medications are very effective if started prior to treatment and continued after.

I am considering having C02 laser resurfacing on my face. I am a redhead with fair skin, age 68 and prone to having cold sores/fever blisters. What can I do to help this not happen after my procedure.
