Sientra does have round implants in the size range that you are asking about, but the implant diameter has to match the base diameter of your breasts. "Extra high profile" is not a term used with Sientra implants, but Allergan's Inspira line does.
Although they are apparently planning to release these in the future, currently Sientra does not have an extra-high or ultra-high profile implants, just low, medium and high profile. Their high profile is actually a little more conservative than the Mentor HP or the Allergan version.
