If you are getting symptoms from your large breasts eg dents in your shoulders/soreness, and it is difficult to exercise then you should have a breast reduction - this will improve your symptoms, and it will be easier to exercise and be active, and you will find it easier to get clothes to fit. In other words, if you are happy with your breasts size, then breast lifting surgery alone may suffice. If you wish the breasts to sit higher on your chest wall and be smaller, then breast reduction surgery is indicated. An in person history and physical would be the best way to decide about what procedure would meet your goals. From the photo alone, either a lift or reduction will likely improve the breast appearance. I lift would not reduce the size of the breasts, so if some of your issues are related to breast size, a reduction would be in order. Thanks for your question, all the best.
I think that in order to make recommendations an exam is essential,. If your breasts are too large a reduction can certainly be performed. If you want the same volume or close to it , then a lift can be performed.
