Facelift and neck lift are separate procedures in very specific group of patients. When we are talking about a pure necklift with a very young patient, it could be because they have experienced a significant weight loss or, have a very specific issue with their neck. With patients over 40 years of age, it is very difficult to seperate a facelift from a necklift because the aging process creates changes in both areas. Also, the platysmal muscle, which is involved in the face and neck, starts at the medial cheek and ends at the collar bone so, it's very difficult to seperate neck and facelift in older patients. In your specific case, you need a facelift which would also involve a necklift . We will be working on the areas of your medial cheek and also on your neck and this would be the best option to address your jowls. So, in older patients when we say Facelift we actually mean face and necklift. One of the most powerful techniques we can use to reshape the jowls and the mandibular line, is a Deep Plane technique.
Generally, with a standard necklift, only the lower part of the jowl bulges, along the jawline and just above or below the jaw will be improved. Any jowl bulging up near the corner of your mouth or above won't be changed. Of course, the procedure can be customized to involve working a little higher in the mower or mid cheek, which would would afford improvement of jowls near the corner of the mouth or above.

Hello, hope you are well. The photo you shared is an unusual angle to assess that area of your face and neck, but generally speaking a facelift must be done to address jowling. Unless it is a relatively conservative neck lift (through a submental incision only), I almost always perform these procedures concurrently (facelift/neck lift) for the best possible outcome. That is why when you meet with your surgeon, it is always best to communicate in terms of aesthetic goals and specific facial features you're hoping to improve on and not necessarily ask for a specific surgery or surgical technique (although it is good to be well-informed!). The neck lift is designed to improve: a double chin, vertical neck bands (platysma banding), excess neck skin laxity, neck wrinkles, and lower jawline definition. The jowl is improved with the neck lift but often times needs a lower facelift or deep plane facelift to achieve the best longterm outcome.
