Is a blepharoplasty and a deep plane facelift the best option for me? (Photo)

I went for 1consultation so far. A lower Blepharoplasty is recommended and a deep plane. He doesn’t recommend fat grafting. had an upper and lower bleph done about 30 years ago, the fat was removed in those days. Is this the best route?

The face ages as a unit. When it comes to facial rejuvenation, any combination of procedures can usually be done that addresses what the patient is most concerned about. It is not uncommon to perform a browlift, upper/lower blepharoplasty, face/necklift with laser at one time to maximize your result with only one trip to the operating room. I would recommend finding an American Board of Plastic Surgery certified plastic surgeon with extensive experience in facial surgery.

Based on your photo, the upper eyelids appear to show more age-related change than the lower lids, particularly if fat was removed during surgery decades ago, which was common at that time. In patients who had fat excised, volume loss often becomes more apparent over time. For that reason, carefully placed fat grafting to areas such as the tear troughs or temples can sometimes restore balance and softness, rather than removing additional tissue.

Additionally, a deep plane facelift would be an excellent option to address midface descent, jawline definition, and platysmal banding. That said, surgical planning should be highly individualized. I would recommend at least one additional in-person consultation with a double board-certified facial plastic surgeon to ensure the treatment plan aligns with your anatomy, prior surgery history, and aesthetic goals.

Thank you for your question.  Although the photo represents a limited view, I would generally agree that a deep plane facelift and lower lid blepharoplasty would add considerable aesthetic benefit. You might also consider an endoscopic brow repositioning and fractionated CO2 laser resurfacing; I would not recommend fat grafting for you. However, when I say "generally" this plan hasn't included a discussion with you, which is vital to making a plan that addresses your concerns and expectations.

It's a little hard to tell from the photo, but your lower lids don't look like they have droopiness or fat hernation so I don't know what a lower bleph would do. Fat transfer would be a good idea, though, to help fill the tear trough as well as other areas of volume loss in the face. A deep plane face and necklift would also work well in conjunction with the fat transfer. Be sure to see a rhinoplasty expert for consultation.

A deep plane facelift will help you with the jowls and especially the neck. However I am concerned that your lower eyes might look hollow after a revision lower blepharoplasty and you also have mild rounding of the lower lateral lid margin.
