I think you have two things going on here:1- it appears as though your nipple is herniated. This occurs naturally on some women and can actually be made more pronounced by augmenting the breast. It is basically a tendency of the nipple to bubble outward beyond its natural borders. Adding a high profile implant behind the breast can actually push the nipple out even further. You can see the deformity very mildly in your preoperative picture, and of course more so in the postoperative picture with the implant pushing from behind. The only reliable way to fix this is with an incision around the nipple which reigns in the tissues which would otherwise want to move outwards. Given that you mentioned that your nipples seemed to be low on the breast mound, at the same time the nipple can be lifted to a new, higher position.2- it's difficult to say from your photographs whether or not your breast implant is riding high or not. If it is riding high, the solution would be to perform further release of that issues at the bottom of the breast. If the breast is in good position but the nipple is simply too low, the fix for that would simply be lifting the nipple at the time of the correction of the herniation. The best person to determine the solution here is the surgeon who performed your augmentation. He or she needs to see you in person to perform an exam and go over your concerns. There are definitely solutions here, it is all a matter of discussing them with your surgeon. Best of luck!Dr. Subbio
