Congrats on your weight loss! Based on limited photos - it appears that you would benefit from a mastopexy/breast lift with slight reduction on a larger side in order to achieve perky breasts. You have enough breast volume - so you may not even need implants. I suggest that you seek an experienced Board Certified PS and discuss your options

Hello and thank you for your question! Based on your photos there is moderate to severe ptosis with the nipple/areola complex far below the breast fold. There is asymmetry in the size and width of the breasts. Based on these photos, in my opinion, you are not a good candidate for a peri-areolar mastopexy and would need an incision that goes around the areola, down to the bottom of the breast fold and across the bottom (anchor incision/Wise pattern). This will be able to raise the nipple/areola complex sufficiently back to a more youthful position and also tighten the breast skin and tissues in the horizontal and vertical dimensions to give you the best results. Fat grafting or an implant may be added to increase the fullness and contour of the upper part of the breast if desired.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Your nipples are located below your breast fold therefore a donut breast lift would be insufficient. A vertical lift would be more suitable for you to reshape your breasts and make them perkier. Implants aren’t necessary unless you want significantly rounder breasts that are also larger.

Hello, thank you for your question. Periareolar breast lift really just moves the nipple upwards however in your case a full breast lift would be need to achieve the "perkier" breast you want. Thank you for the photos. Judging by the photos you have posted, the best results would be obtained with a full lift. A donut mastopexy and a vertical mastopexy would not suit you well. Also you have some breast asymmetry that would need to be addressed at the time of surgery by removing some breast tissue to hopefully try to place the same size implants or close to it.
I would recommend you see a board certified plastic surgeon and make certain to get 2-3 opinions. Make certain the surgeon you see has great deal of experience in breast implant and breast lift surgeries.

Actually, you would benefit most from an anchor pattern scar (also called inverted T). This would give you the maximum lift. Please visit with a board-certified plastic surgeon. Good luck!

You need the vertical lift to better shape your breasts and remove the excess skin. You have too much sagging for the peri-areolar lift.

A Benelli "doughnut" mastopexy is almost NEVER the right answer. Very unpredictable results and bad scars are the norm. You pictures suggest a vertical scar approach might work, but it could require a short T or J shape for skin excess (always hard to tell for smaller cases). I think short T scars look better if you weren't getting an implant to take some of the slack out
