Doctors are human and as such they vary just as much as anything else in life. Different plastic surgeons will have different opinions, different backgrounds in training, different amounts and types of surgical experience, different number of years in training, inherent different skill sets and different personal philosophies. Delivering consistent high-quality liposuction and fat transfer results is more difficult than most people believe. Assassing how much fat can be removed on a low BMI patient accurately it’s probably one of the most difficult skills we have as plastic surgeons and even for those highly experienced it can still be daunting. Weight, height or BMI helps significantly in our understanding of how much body fat someone has though pictures is what tells us their fat distribution. Still, there’s no substitute for having an in person examination to get a quality and accurate assessment. For most BBL procedures fed is harvested on the abdomen and flank area. personally I view the torso as a single anatomic unit and try to treat the entire torso if I’m going to do Liposuction. The transition zones that spot treating the torso leaves are signs of lesser quality work and in my opinion should be avoided. When it comes to the goal of the procedure for most BBL patients my goal is to create smooth continuous accentuated S-curves from their armpit to their knees. when reviewing results make sure you’re seeing the entire torso and most of the lower extremities in the pictures. do the patients look natural and attractive in the pictures? some providers purposely do liposuction of the upper thigh to try to emphasize the buttocks being larger. doing so destroys the S-curve that is the hallmark of female beauty. I suppose it’s seeing the tree and not the forest. make sure you view results from all angles including front, three-quarter, side and back. The three-quarter angle views tend to be the most telling of quality results. Before and after pictures should include the entire torso and the lower extremities at least as low as to the knees. On a female torso treating the “full torso” includes upper abdomen, lower abdomen, love handles, waist, full back and under arms. On a male torso it would also include the chest. For patients who may not have enough fat for an ideal fat transfer procedure it is generally better to include more areas to harvest fat from then encouraging patients to gain weight. rarely if ever does somebody look better gaining weight and if someone is at a healthy weight is highly unlikely they’re going to maintain the increased weight long-term just to maintain their BBL results. If liposuction of the full tourso is insufficient for a quality fat transfer procedure then the next go to area is typically arms or inner thigh. of those two arms generally gives higher patient satisfaction and better aesthetic outcomes but inner thighs can be an excellent source to harvest fat if more fat is needed. In my opinion treating your full torso and doing that well should be enough to do a quality BBL in your case. if more fat is needed I would probably consider adding arms but obviously this should all be done in context of having an in person consultation and listening to the patient’s wishes. I said this earlier but I’m going to say it again. delivering consistent high-quality Liposuction and fat transfer results is more difficult than most people believe. Results from these procedures are permanent and irreversible. I cannot emphasize enough the importance of finding the most talented an experienced provider for both Liposuction and fat transfer procedures. Doing that is no easy task but the following are some general guidelines for how to find a quality provider. 1. I recommend patients avoid traveling long distances for elective surgical procedures. There are of course instances where there is a lack of local talent and traveling to some degree is inevitable. If traveling for the procedure patients should be ready to travel to have the consultation, post operative appointments and follow up should there be issues or complications. Patients sometimes do not appreciate the importance of in person consultations, that complications do happen or the number of people who have issues with their results or need revisions. 2. Schedule multiple consultations with different providers in your area who seem to have a good reputation for the procedure you’re interested in. Starting with at least five consultations seems like a reasonable number to me. 3. Whenever possible schedule in person consultations. 4. During in person consultations ask each plastic surgeon to show all of their before and after pictures for the procedure you’re interested in. The goal is not to see the best results of their career but to get an idea of what average results look like and get some type of confirmation of how many of these procedures that provider has performed. Ask to see results that were not as good as the doctor would’ve hoped for and talk about the differences in outcomes. Honest and experienced providers should have no problems presenting this type of information. A highly experienced plastic surgeon should have hundreds of before and after pictures for the procedure you’re interested in. At a minimum at least 50+. 5. Read all the reviews on various physician review websites paying close attention and avoiding providers with any significant number of justified negative reviews. Providers with an abundance of reviews often have aggressive marketing campaigns. The plastic surgeons I respect most all have the single variable of not having justified negative reviews on their online profiles. In my opinion justified negative reviews are far more telling than an abundance of positive reviews. 6. Avoid scheduling for surgery or making a deposit on the day of the consultation. Give yourself some time to think about the decision and make sure you finished all scheduled consultations before committing to having surgery that will permanently and irreversibly change your body. When in doubt slow down and schedule more consultations. Choosing the right plastic surgeon is the single most important variable to having a positive experience. 7. Don’t go chasing after technology or names of certain equipment. Instead focus all your efforts on finding the most experienced and talented board-certified plastic surgeon. The BBL procedure can gather spectacular body transformations that can border on miraculous. You can also leave people permanently disfigured. The difference in outcomes is purely related to patients who were able to find quality providers who had sufficient skill and experience to deliver quality results on a regular basis. Find the right plastic surgeon and everything falls into place. Every community has talented and not so talented providers. Exhaust the local community before hopping on an airplane.
