Rhinoplasty- Deprojection vs. Supratip Break, can both be done?

In general it is usually possible to do both tip deprojection and supra tip break creation.

It's hard to say for certain if this is the case with you without seeing your nose, though.

Generally speaking, if the anatomy allows for it, you should be able to achieve both. By deprojecting the tip, you bring it more in line with the level of the bridge. This would tend to diminish or eradicate a supra tip break, unless the bridge is further reduced. You should be able to see similar examples of prior work from any surgeon you go to for a consultation.

Rhinoplasty requires exceptional skill on behalf of the surgeon and great communication with the patient about realistic goals and expectations. Common cosmetic concerns include a dorsal hump, wide nose, sunken nose, irregularities, and asymmetries. Functional concerns include difficulty breathing through the nose, which can and should be addressed at the same time as your rhinoplasty.

In your situation, both are definitely possible. A supratip break has to do with the relationship of the dorsum to the tip, where as deprojection usually refers to the tip only.

Make sure to seek out the care of a board certified surgeon with extensive experience in rhinoplasty. Find someone you connect with, who will listen to your concerns and provide you with a framework for what is possible.

Thank you for your question. You should be able to get both -deproject the nose and get a supratip break. Make sure you you only seek opinions from very experienced rhinoplasty surgeons and those that perform at least 6 per month. Evaluate their on-line galleries critically. Best wishes to you.

You are the first patient I have ever heard who wants a Supra beak deformity. It is possible that you aren’t clear on what a suprabeak is. It would be worth getting this clarified. If a patient asked me to create a suprabeak I would refuse because I know that person will be unhappy.

Without reviewing the precise anatomy of your nose, it really is not possible to voice an opinion. It sounds as if you may be discussing performing surgery only on your tip, in which case it may indeed not be possible to have both. If however you are having surgery on the entire nose and therefore the nose above the tip could also be reduced, then it may indeed be possible to achieve both. I would suggest continuing this conversation in more detail with your surgeon so that he or she could better explain to you what the conflict is in your case.

A full set of facial photographs from all angles would be helpful to make a determination about how best to proceed. Rhinoplasty is performed 3 dimensionally, since the nose is a 3-dimensional structure. All the components of the nose must balance with themselves, and the entire nose must balance with the patient's facial features. Depending upon the thickness of the skin in the supratip area the nose, the nose can be de-projected somewhat while creating a supratip break. Patient's with thick skin tend to require steroid shots in the supratip area of the nose to give that break.

Thank you for your quesiton. Both are possible, as they are two different maneuvers, although one may affect the other. Deprojection refers to how far your nose sticks out (farthest point of your nasal tip) from your face when looking at your side profile. Supratip break goes with rotation of your nose and refers to a gentle depression just above the tip of your nose to create a more "lifted" tip from the line of your dorsum. Surgeons with knowledge and experience in rhinoplasty can help address your goals and discuss with you on what is realistically achievable.
