39 year old female. When I smile I get much more bulging under one eye than the other. I already have Botox around my eyes which helps to reduce the bulging, but I am still very self-conscious of it, and I am concerned that it will worsen as I get older. Is there anything that can be done to correct/improve this? Thank you.

The change in your eyelids were new smile is caused by the muscle contracting underneath the eyelid. You do not have I bags. Surgery cannot correct muscle fullness. You might try Botox in the crows feet area with more being placed on the left side.

Answer: Asymmetry All people experience some degree of asymmetry and I would submit that the amount that you have appears natural. You can treat the area with a conservative amount of Botox to soften the muscle activity around the eye. That may give you more of the symmetry you desire. I recommend using an experienced injector for this sensitive area.

Answer: Under eye asymmetry Dynamic bulging occurs when smiling and is due to contraction of the orbicularis muscle. I would consider treating with botox, which has already been performed. Dermal fillers can also be applied to help improve the asymmetry.

While I understand your concern with this dynamic asymmetry, I do want to reassure you that some bunching under the eyes with smiling is normal and actually somewhat natural appearing. This is caused by contraction of the muscle that lies just under the skin of the eyelid called the orbicularis. If the amount of bunching is bothersome to you, you can elect to put a small amount of filler, for this area I'd recommend Restylane or Vollure, into the area that has less bulging to try to even the two lids out. Hope this helps!
