Modifying those cartilages is key to making the desired changes. Usually, when a tip is wide like  yours, there are other issues as well. For example, the same tip cartilages that make the tip look wide often make the nose look too long on profile view, and/or they make the tip appear to project forward from the face too far.You should know that modifying a tip requires advanced techniques that not all plastic surgeons can handle. You must see before and after photos of any surgeon you are considering. Reduction of a bulbous tip is one of the most commonly addressed problems in nasal tip rhinoplasty. Excess cartilage can be removed, the cartilage can be repositioned in a more narrow fashion, and additional cartilage can be placed on your tip to provide improved tip definition. A tip rhinoplasty in which the domal cartilages are reduced in size and sutured together will correct this.  A small cartilage graft can be added to the tip as well for more definition.
