This response was dictated. My apologies for any potential grammatical errors. These type of lower extremity or buttocks indentations from certain childhood injections are not uncommon. The true ideology of the indentation is not always clear, but most likely it is a twofold problem of fat necrosis, and the formation of scar tissue that happened from an injection of locally noxious material. most likely the injection was meant to go inside the muscle that can handle the injected, chemical much better than adipose tissue. The problem is as mentioned, usually twofold so simply adding volume by itself, may only give a partial improvement. It may be worthwhile to try one round of fat, grafting, recognizing that it may still leave some deformity, but should give an improvement. Usually complete resolution with will require more than one procedure and may require releasing scar tissue as well as grafting fat. This type of work is not easy or straightforward. To increase your likelihood of a quality outcome, including an accurate assessment, I suggest focusing your efforts on provider selection. Look for plastic surgeons, who have ample experience and proven track record with fat grafting. During each consultation, ask each provider to open up their portfolio and show you as many before and after pictures of restorative fat grafting as possible.Look for providers who recognize the complexity and challenge treating these problems. Providers who seem overconfident and promise complete resolution with a single treatment should be asked to show extensive collections of before, and after pictures of the previous similar work. There is one very important caveat that needs to be understood before considering this treatment and selecting a provider. Early fat transfer results can look very impressive. They can look too good to be true in fact. That said keep in mind that early fat transfer results do not represent final long-term outcomes. Whenever reviewing before and after pictures of any fat grafting, a.k.a. fat transfer procedure always get confirmation of the timeframe of when after pictures were taken. Results can be considered final 3 to 6 months after the procedure. Photographs taken earlier than that are not representative of what the procedure can or cannot accomplish. Both Patient and providers may be tempted to show off quality work by displaying early post operative pictures that look much more impressive than the final outcome. Finding the right provider may require having quite a few consultations. Look for plastic surgeons in your community who seem to have a lot of experience with fat transfer. Best Mats Hagstrom, MD
