Hello everyone! I hope you can help me. I've always struggled with my side profile. Not only do I have a larger, crooked nose but I also believe my strong chin and jaw structure & also bigger bottom lip creates an uneven & unfeminine side profile. But perhaps I'm wrong. I would love to have a more appealing side profile. What would be your recommendation to help with this without multiple surgeries and risks? I would appreciate your advice greatly. I'm 28 years old with no history of surgeries
Answer: I think your jaw and chin are great. See the morphs I made of your nose. Click on the "Web reference" link, just below my response, or go here: FacialSurgery.com/RealSelf/RealSelf_CaroAlert.html I made a couple of computer morphs of your nose, and animations of the morphs, to show the changes that are possible for your nose in truly expert hands."More feminine" usually means "smaller and less prominent" when it comes to the nose, so in the modification I narrowed your tip, lowered the bump on the bridge, elevated the tip, and then brought the entire nose back closer to your face. I paid special attention to lowering the height of the bridge up where it begins, just below your eyebrows. If that area isn't lowered, then after surgery your profile line would look like it slides from your forehead right onto your nose, without a little dip below the eyebrows, like a Roman statue. That's not a good look. You should understand that the changes I demonstrated in the morph require advanced techniques, techniques that most plastic surgeons cannot handle. Be sure to read the section in the "Web reference" link on how to stay out of trouble while searching for a rhinoplasty surgeon. I'd love to hear what you think of the morphs. Did they seem to address your main concerns? Your nose is also a good example of why computer imaging is mandatory in rhinoplasty. You need to know exactly what the surgeon is planning to accomplish -- what features he thinks he can change, and by how much he thinks he can change them. When you see his goals, you'll know whether he has an eye for a beautiful nose, and whether he shares your opinion of what constitutes a beautiful nose. You'll also know whether the changes he proposes are enough to be meaningful to you, and whether he understands your wishes enough to address all of your priorities. But remember, you're not hiring him for his skills with the computer. The doctor must then show you his before and after photos to prove that he can actually accomplish what he draws on the computer. In your case in particular, you need to find a surgeon who understands all of the issues I mentioned, and how they fit together, and who can actually accomplish them in surgery.

Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. You may also benefit from a nasal tip deprojection and a conservative dorsal hump reduction. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Answer: Rhinoplasty I agree with the others that your chin and jaw structure is aesthetically ideal. I do believe that changes to your nose would create a better balance between your nose and existing facial structures.

Answer: Advice on strong and un-feminine side profile? A rhinoplasty procedure can accomplish making the nose look more refined by shaving down the dorsal hump and straightening the crooked nose. Best to leave the chin alone. Rhinoplasty is a very difficult endeavor, so choose your surgeon very wisely based on extensive experience. Digital computer imaging performed in the office setting can also give you a very close idea is to what your new nose would look like upon your facial features. Answer: Challenging case, my comments: To begin with, I disagree when you don't consider having a feminine profile, it is actually very feminine and this is not your issue; however it is true you can get a great improvement with both, rhinoplasty and osseous genioplasty (profileflasty is called when done in combination); I'll explain furter. Yours is a basic rhinoplasty case, to be done via endonasal, no external scars, no grafting needed, to apply the basic set: -dorsal shaving -osteotomies to close the roof and narrow the bones at nasal base -slight tip deprojecting (medial cruras trimming only) -significant trimming of cephalic edge of lateral cruras and domes of the alar cartilages, in lay words: reducing and feminizing the tip, within proportions Low difficulty case, 5-6 out of 10 scale, surgical time 60 min in expert hands. Your chin needs a very specific and atypical / custom design of a three-dimensional osseous genioplasty:-it is too prominent, needing slight / moderate deprojection by means of interstitial bone reduction (never ever backwards sliding, which leads to a hanging double neck) -too short vertically, disproportionately small from lip to edge, needs a slight vertical increase -too sharp, "withche's" like, due to a narrow horizontal dimension in the frontal view; this can be rounded by means of a slight interpositional graft to widen the apex of the chin This operation needs a lot of experience with ossous genioplasties, very very good aesthetic judgement, lots of discussion with the patient and a meticulous antropometric preop planning. Do research well experience of the surgeon, the results are awesome and the procedure safe, however your design is not a routine one, need very good "eye".
