I lost 100 lbs abojt 4-5 yrs ago. I mannaged to gain some back and working on losing 20 more lbs. Im not sure how much of it is fat vs skin. Do i need a tummy tuck?

If you've lost 100 pounds and you have loose stomach skin, the best surgery for you would either be a tummy tuck or a 360 circumferential lower body lift. Other surgeries like liposuction will only remove fat and leave the stomach skin even looser. Unfortunately, with your weight loss, your skin is not likely to snap back with only liposuction. Therefore, most plastic surgeons would agree that in a patient with a 100-pound weight loss with loose stomach skin, you would do best with a horizontal abdominoplasty or tummy tuck. During a tummy tuck, an incision is made low in the pubic region and then extends over the hips. Tissue dissection is done up to your sternum, and the rectus abdominis muscle is exposed. The rectus muscle is plicated because you have lost weight and the fascia is weak. Then, the excess skin and fat from the top of the pubic region to just above the belly button is removed. The skin that is at the level of your belly button is pulled down to the pubic region. The mons pubis can also be reduced during the tummy tuck. A tummy tuck takes approximately 3 to 4 hours to complete under general anesthesia. Plan on a 2 to 3-week recovery. Drains will need to stay in place for approximately 10 to 14 days. First off, congratulations on losing all that weight, that's definitely a big accomplishment! From the pictures you have provided, it is honestly difficult to make out the details of your abdomen well enough to advise you one way or the other. All I can tell is you have some excess subcutaneous fat and skin diffusely throughout your abdomen and flanks. In general, after patients lose a significant amount of weight (as in your case), their skin is not able to fully stretch and recoil properly, and therefore usually does not fully contract with just liposuction alone (without resecting any of the excess skin)- that tends to produce a very deflated appearance and ultimately requires another procedure to resect the excess skin and tighten the overall contour. This is why those patients usually benefit from an abdominoplasty (tummy tuck) and usually also liposuction of the flanks to best help improve their abdominal/trunk contour. With all that said, it is always difficult to provide tentative surgical recommendations, as there is only so much information to be gained from any photos and descriptions provided. I recommend for you to come see me or another plastic surgeon for a formal consultation to thoroughly discuss your surgical goals, undergo a formal examination, evaluate all your options (surgical and nonsurgical), and decide on the best procedure(s) for your specific case.
