Please see the attached photos. I have a very saggy, masculine looking face at 27 years old. This has been an ongoing issue for a long while, as you can see in the provided photos I have jowling which makes my face appear masculine and square. A facelift seems to be the only option to lift my jowls and provide me with a more feminine, oval and soft face.

Anti-wrinkle injections (Botox) in the masseter muscle can help ease the tension in this area, transforming a broad jawline into a more defined, sleek facial contour. Thank you for your question and for sharing your photographs. Before considering surgical interventions, I suggest trying botulinum toxin (Botox or equivalent) as a solution to reduce the width of the mandible at the angle of the jaw (gonial angle). The masseter muscles tend to produce considerable prominence and a square appearance as they wrap around the lower border of the mandible, increasing not just the width but also slightly the height at the gonial angle.

In addition to facelift surgery, you can also discuss with a maxillofacial surgeon the option of mandibular reduction if the botulinum toxin treatment is not adequate. Finally, and probably too early for a 27-year-old, to understand what the facelift procedure can achieve for you, I suggest examining your appearance in a mirror when you lie down completely flat without a pillow. This gives you a reasonable representation of the impact of a deep plane facelift. My suspicion is that you might be better off with different solutions rather than jumping to a facelift at this age.

It looks like you may have heavy masseter muscles that could be addressed with botulinum (botox).  This is an in the office injection.  If that doesn't do it, we can consider narrowing the jaw bone and tightening the skin. A facelift is not likely the best way to go for you.
