Is it safe to have a deep plane facelift after radiation treatment ?

Hello, I am 52. I am interested in having a deep plane face lift. When I was 21 yrs I had radiation treatment on my neck for Hodgkin’s Disease lymphoma. I understand that this would have caused some scarring. I also had PDO threads one time last summer. Have you or any surgeons you know done a deep plane face lift in this case? Any research on topic? Would it be too risky for a successful healing? Please let me know any concerns of things to think about. Thank you! Tam

Radiation has long term effects on vascularity to the skin, adversely effecting endothelial cells in the microvasculature.  However, the density and collateral vascular supply to the face and neck is very dense.  Can you have a deep plane facelift after radiation? Yes, in experienced hands with some modifications in technique.  Are you at increased risk of adverse healing events because of your radiation therapy?  Yes you are, but not overly so.  Having anecdotal experience with such patients, supported by the surgical literature, this is exactly the advice I would give you in consultation.  However, I would avoid any laser resurfacing (low energy) to the neck.

There are studies about skin changes after radiation, the most important fibrosis and changes on vascularization.  This changes make a difference on any surgery since cause the different layers  are stick together and can have wound healing issues. A personal evaluation on your specific case is very important to check the tissue quality and the area as well.

There is plenty of information on the internet quoting studies of facelifts in patients with prior radiation. I think it’s worth reviewing some of them and going on a few consults. There is increased risk if wound healing problems but it varies on a case by case basis

Hello, hope you are well. This is a great question, but it is highly individualized to the patient and surgeon. Radiation can leave varying degrees of fibrosis and changes to the skin quality (partially dependent on the administered dose) that may exclude some but not others from elective surgery. It often continues to worsen for years afterward. In any case, a history of radiation would be considered a risk factor for wound healing complications. However, as to whether it would be 'too risky' can only be assessed in person on an individual basis with a detailed physical examination.
