Thank you for your question. A large portion of my practice is dedicated to revision breast surgery. You do have mild symmastia. It is a straightforward fix in experienced hands. If you have confidence in your surgeon and he is experienced at breast revision surgery, in particular symmastia, I would have him fix this. You should be able to have an excellent result. I don’t think you have true symmastia, but you’re implants are larger than the natural diameter of your breast and that volume has to go somewhere.At this point, you can either live with the results, considering decreasing the implant size, or have a surgeon attempt to close the medial aspect of the implant pocket. Partially closing, the implant pocket is technically challenging, and should only be attempted by those who have experience and success doing that procedure. Ask surgeons to show you before and after pictures if considering revision surgery, other than simply downsizing your implants. to make an accurate assessment on the outcome of a plastic surgery procedure with generally always need to see proper before and after pictures. You did not post photos, which would have been helpful However, the lateral pockets can be tightened, and you could go down in size so that the circumference is smaller.
You might prefer the smaller size and your surgeon could tighten your lateral pockets so they don't move as much.
If you are bottoming out, you do need to go smaller. Most can drop down up to 150 cc's and still not need a lift with internal lateral pocket tightening.

Most will probably be high or ultra high profile, especially to give the round fake ball look. Please do not go too large for your frame and tissue.
Don't go any larger since you are starting to bottom out from the weight that you now have. You can try changing them to a higher profile and use mesh for internal support. Larger implants will sag down even more over time.
