It is difficult to answer your question with the single photo you have submitted but it appears that you have minimal if any sagging and good quality tissue at this time. This suggests you could substantially reduce your implant volumes without causing excessive sagging.

You will need smaller implants and a circumareola approach to excise the excess skin. You will not need the anchor scar.

If you are bottoming out, you do need to go smaller. Most can drop down up to 150 cc's and still not need a lift with internal lateral pocket tightening.

Unfortunately large (i.e. heavy) breast implants in very thin women tend to sag, bottom out, and age more quickly. There are factors that would affect your ability to downsize your breast implants and the possible need for a breast lift. Some of them include your age, health, skin quality, original breast augmentation date and others. Only your plastic surgeon knows those factors and would probably be the best to address the question. Good luck!
