Question

I did BA with 330cc sientra HP( width:11.7cm, proj:4.7cm) I like the width on me but I didn’t like the projection, I’m now B cup! or very small C (C little bit loose), 330cc didn’t add too much change for me, I was AA and now I’m B, I want more projection and I don’t looking for natural looks, the Dr. suggested 445cc sientra UHP( width:11.5cm, proj:6cm) or 470cc sientra UHP (width:11.7cm, proj: 6.1cm), is 115cc increase in size for 445cc UHP will make a noticeable difference or 140cc?

Don't go any larger since you are starting to bottom out from the weight that you now have. You can try changing them to a higher profile and use mesh for internal support. Larger implants will sag down even more over time.

With the sizes, your mentioning you will increase the augmentation by about a third. Whether that’s sufficient for you is a decision you when your provider needs to make. Using excessively large implants leads to more of an unnatural appearance, increases undesirable, secondary, side effect, complications and increases revision rates.
Implant choice is something that should be decided between the plastic surgeon and the patient. Is it an important part of the decision and you should look for your provider for guidance for these decisions. Each provider will have their own way I’ll be determining what implants to use. Some providers make the decision themselves while other providers leave the decision exclusively up to the patient.
 if you want proper second opinion consultation, I suggest scheduling those as in person consultation coming, prepared to bring with you a complete set of proper before and after pictures and a copy of your operative report.
