Hi RealselfnewbieTX. The difference between the Natrelle Inspira Responsive and Cohesive is the cohesive level of the silicone gel inside of the implant. Some people like to refer to it as different levels of gumminess. When I use the word "cohesive", I am referring to the cross linking of the silicone gel inside the implant. The more cohesive the implant, the better it maintains its overall shape when held upright in your hand. With the Natrelle Inspira collection, there are 3 different levels of cohesiveness: Responsive, SoftTouch, Cohesive. The Responsive implant is the softest and least cohesive. The Cohesive implant is the most cohesive with the most cross links. The SoftTouch implant is an in between option, which is a great for patients who don't prefer the firmness of the Cohesive implant, but want to have the benefits of the extra cross linking. Each implant is great for the right kind of patient.I commend you for being so thorough and doing your homework. Make sure that you look at plenty of before-and-after photos and that you're thoroughly impressed with your surgeon's work. Your surgeon should also be able to let you try out the different implants to see what feels right to you. If you have any doubts, keep searching. There are many reputable, board-certified plastic surgeons in your area, and you will not have a problem finding one whose work and personality are the perfect fit for you. Best of luck!
All silicone implants have cohesive gel, which means that it holds together as a semisolid. The first Inspira implants had a soft gel but a higher fill percentage than the previous line of implants. Soft touch (paradoxically) is a firmer gel but the same fill percentage, and the newest cohesive gel is the same as used in "gummy bear" implants but in a round shell. These are quite firm and are used when more upper pole fullness is desired (a less "natural" look) but they have the least tendency for rippling.
Silicone in general can exist from a liquid to a solid and varying degrees in between. The initially designed silicone implants were a thicker solid shell with a more liquid inside. Many variations have been tried giving us the variety of choices we have here now which allows plastic surgeons the ability choose the best implant for their patients. The answer to your question is that the Responsive gel, also called Trueform 1 is a cohesive gel that yields a very nice result in women with some soft tissue to cover the implant. The cohesive (Trueform 3) is the same firmness as what has been called the "Gummy Bear" implant which gives a firmer result which is less likely to ripple and wrinkle and does well for breast cancer patient. Allergan launched in the US the SoftTouch (Trueform 2) just this past 3 April 2017 rounding out the Inspiration Collection best for patients with thinned tissues in a primary or revision case.
