Is my eyelid hooding caused by orbital fat loss or are my eyelids hooding due to age? I'm 34 years old. Will filler correct this or will I need eyelid skin removal? It looks as though my sulcus is really hollow but I'm not sure. Please tell me what you see when you look at the image. Thanks
Answer: Upper Lid Hooding Thanks for your question and for posting a photo. The hooding that you point out seems to be due to upper lid skin excess- known as dermatochalasis. This is commonly seen as one ages. The optimal treatment is an upper lid blepharoplasty which can remove excess skin, and is done quite easily in the office under local anesthesia. Fat can sometimes be removed as well, though I personally tend to avoid it unless necessary so as to not cause a hollowing out of the upper lid. Fillers or fat injection are sometimes offered as well to add volume to the upper lid in cases of hollowing. Seeing a board certified or board eligible plastic surgeon, facial plastic surgeon, or oculoplastic surgeon will allow a full evaluation. Being evaluated for ptosis and the integrity of the levator muscle- which lifts up your upper lid- is important prior to embarking on upper eyelid surgery. Answer: Sulcus You do have a deep upper orbital sulcus on both sides with some excess skin. An upper lid bleph to remove a crescent shaped area of skin from the upper lids with micro fat grafting of the upper orbital rim would look nice.

Answer: Eyelid hooding Hi and thanks for your question. You may have a lot going on with your eyes. It's hard to tell without a profile view about the position of your globe (eyeballs). Irrespective of this you do have excess upper eyelid skin which would benefit from an upper blepharoplasty. Your upper eyelids do appear a bit hollow and upper blepharoplasty can help restore some volume if your surgeon does a very limited muscle excision. Sometimes filler can also help but it's pretty risky in you because your so thin. There wouldn't be much room for error and you would be a setup for lumps and bumps. Hope this helps!
