Options for improving lower face? (Photo)

Question

I'm only 40 but have some drooping around my mouth and jaw area. It bothers me because people will ask why I'm in a bad mood when I'm actually fine. I tried PRP but had limited results. Curious if I'm a candidate for any surgical options. Thank you in advance.

Judging from your pictures, it appears that you are an excellent candidate for a closed elastic lift, given the slight sagging you have, along with a facial fat transfer procedure.

Thank you for your inquiry regarding the drooping around your mouth and jawline. Based on the image you provided, it appears that there are relatively discrete changes in the laxity of the lower face and along the jawline.

Here are some options to address these early signs of drooping:

1.	Volume Augmentation: One approach to camouflage the laxity is through volume augmentation above the area of concern. This can be achieved using Sculptra or fat transfer. Both methods can add volume and provide a lifting effect, which helps to improve the overall contour of the face.
	2.	Tissue Tightening Procedures:
	•	Thread Lift: A thread lift is a minimally invasive procedure that uses dissolvable threads to lift and tighten the skin. This can provide a subtle improvement in facial contour without significant downtime.
	•	Minimal Access Facelift: This procedure involves a small incision placed in the sideburn area and is suitable when the skin has reasonable elasticity and the changes required are modest. It can effectively tighten the tissues and improve the appearance of the jawline with minimal scarring.
	3.	Facelift Surgery: While a full facelift surgery is the most comprehensive option, it may not be necessary at this stage given the early nature of your concerns. However, it remains an option if more significant tightening and lifting are desired in the future.

I recommend a lip lift.  A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners.

I have been cautioning my regular followers for a long time already about PRP treatments. So, I am not surprised at your disappointment with them. From the photos provided, there appears to be volume deficit in the cheeks, as well as lack of definition of the jawline. In the hands of an experienced injector, both of these problems can quite easily addressed with a robust volumizing HA filler for the cheeks and a robust HA contouring filler for defining and strengthening the jawline. The results are typically immediate and engender an immediate "Wow!" reaction. You would be wise to consult with an experienced, board certified cosmetic dermatologist filler injector before jumping into invasive, aggressive and expensive surgeries that also involve prolonged post-operative downtime and the risk of scarring.
