Best procedure for jowls? Lower facelift/mini lift? (Photo)

Question

I’m 46, reasonably healthy, non-smoker. I have regular Botox (glabella, mentalis) and small amount of filler in cheekbone area. I have a round face and have noticed some jowls and sagging. What would be the best procedure for me? I’m considering a mini facelift or lower facelift.

Conducting an examination to assess skin laxity is a priority in facelift surgeries. I prefer performing a mini facelift primarily for the mid-face. For the lower face and jawline, a lower facelift is recommended. If there is significant sagging in the neck and submental area, I combine neck lift with lower facelift. You appear to be a suitable candidate for the lower facelift. The fact that you don't smoke is a significant advantage for your recovery process

Based on your areas of concern it seems that deep plane short scar facelift intervention would be most suitable.

To get a sensible idea about the impact of a deep plane facelift I typically suggest two prospective patients to examine their facial appearance lying down completely flat without a pillow. It produces a reasonably close representation of the impact of a facelift intervention.

If you feel that this is the type of change you wish to achieve, it would be a matter of discussing with a surgeon that specialises in this type of interventions.

I have found that the deep plane facelift is more consistent with the degree of improvement achieved for the jawline contouring and marionette lines.

A mini facelift with a vertical plication of your SMAS layer should help to elevate your jowl and straighten your jawline to give you the results that you’re looking for. This is frequently performed under local anesthesia with sedation to minimize cost.  Best wishes.

Hello, hope you are well. Thank you for posting your photo and question! It would be useful to see a profile view to have a look at your chin projection, but based on what I can see here your best investment would be with a well-executed facelift. An even more precise recommendation would be a modified deep plane face lift to target the jowling. You could consider energy-based minimally invasive treatments, but I do not think you would find the results satisfactory if your goal is to eliminate the jowling entirely. A mini facelift would provide you with scars, but not adequately address the jowls and jawline.

It appears from the photo that you could have a small chin and that is contributing to the jowl prominence. Otherwise you have good skin, firmness. It might be better to start with a chin implant and down the road a facelift. When you decide to do a facelift, get a Deep Plane Facelift for a longer  lasting effect.

For the purpose of lifting your face, you could consider the surgical or filler route. However, regardless of what you decide, you should consider filler in your lower middle portion of your face to support your marionettes, chin, middle jawline area as well.
