Question

I had a lift (regret) and implant exchange for right side bottom out snd left side rupture. I have these odd contours below the nipple and no infirmary fold. Is there a way to exchange implants (downsize) and define the fold? My nipples are at 19cm from the sternal notch snd I don’t want another lift. Current implants look too big, stuck on and little natural movement. They are mentor xtra

Thank you for your question and pictures.  From what I can tell you had a vertical or lollipop lift and often times this is the shape result.  In order to correct this, with or without changing to a smaller implant you would add a scar at the inframammary fold area to correct the shape and the 'Lack" of IMF. Thank you for your question.  I have been in practice for over 30 years, and revisionary breast surgery is a large portion of my practice.  I have written several papers on this topic.  You are a very simple fix with an internal bra/capsulorrhaphy and excision of inferior pole skin.  You should be able to have an excellent result.  Best wishes to you.

Loss of the inframammary fold (IMF) after a prior lift and implant exchange is usually a structural issue, not just an implant size issue. When bottoming-out occurs or the fold is over-dissected, the natural boundary that defines the lower breast is weakened, which creates that “stuck on,” heavy, unnatural contour you’re describing. Simply downsizing implants alone will not reliably recreate the fold unless the pocket and fold are surgically reconstructed at the same time.Yes, it is possible to downsize and redefine the fold without performing another full lift&mdash;especially since your nipple position (19 cm from the sternal notch) is already acceptable and you don’t want them higher. The correction typically involves precise pocket revision with internal fold reconstruction (capsulorrhaphy and fold fixation) to re-establish a stable inframammary boundary. In some cases, using a slightly lighter or better-proportioned implant improves natural movement and reduces that “stuck-on” look.
The key is not more lifting, but restoring structural support at the fold level. When done properly, this can improve contour, define the lower pole, and create a more natural transition without unnecessarily revising the nipple position.
