Thank you for your questions. As for hiding the scar over time the weight of your present implant would start to settle the implant into a lower position and start to hide the scars. As for the new look size, you would probably need at least 200 more ml of implant volume to achieve that look. making sure that the width of the new implants is wide enough to result in more cleavage.

Thanks for your questions. Based on the picture you might have a mild Pectus excavatum, but this should not affect too much your results of a breast augmentation. What might affect the results to a small degree is your slight breast size and location asymmetry, but again you should be able to get a very nice result. From what I can tell you had a breast reduction only on the right breast and now the left breast sits lower and more lateral. This should be correctable with a lift on that side. Be sure to see a Board-Certified PS for your consultation and possible surgery

Thank you for your question and picture. That might be the correct new size of implant. The best way to tell is to have your surgery use temporary sizers in a bra in the office ADDING SIZE to the smaller size until they both look equal. Then the PS will have a better idea how much smaller to make the larger size. And then, I would still use temporary sizers in the OR to be sure to get the closest symmetry possible.

I would not try to move that scar to the areola border as it will likely distort the areola. As for the scar, and depending on how old it is, it will most likely fade over time if it is relatively new. If it is raised, or a keloid, it would improve with steroid injections and finally if it is a flat and old scar and then won't likely change anymore, then scar camouflage medical tattooing could be done to blend the color more with the surrounding skin.

Congratulations on your impressive weight loss, the hard way too. Any more weight loss would just create more loose skin. At this point the abdomen would be addressed with various different types of abdominoplasties which would best be determined on direct physical exam. Either standard or Extended TT, Fleur de Lis TT or circumferential TT. As for the chest, likewise you will need mostly lots of skin removal although there may still be some breast glandular tissue present which could be determined on exam. Both of these surgeries could potentially be done the same day under the right conditions. Be sure to see a Board-Certified PS for your consultation, and in my opinion, that operates either at a hospital or hospital affiliated surgery center.

While a FDL 360 will correct better that vertical looseness that your picture demonstrates, I believe that with good distribution of the skin during closure you could achieve a decent result and still avoid the vertical scar. Which, by the way, could be added in the future to convert the TT to a FDL TT if necessary. Be sure to see a Board-Certified PS for your consult.

From what I can see, and again being difficult without an in person exam, I do believe that you would be a good candidate for and in need of a back lift, or bra line lift for the skin excess. As for the upper abd skin excess, I would be comfortable with additional skin excision using probably only the vertical scar. I would reserve a dual plane approach for those patients where you are trying to correct a very modest amount of ptosis or droopiness which you do not have. In your case I would likely use a moderate high profile Mentor gel implant either in the XTRA or Boost variety to get a nice natural appearance with less risk of the subpectoral implants not settling into place which potentially has a high chance with the Motiva Ergonomix 2

It all depends on how long ago the surgery was. If recent then you might need to do some massages to have the pockets loosen and the implants fall into place. If it has been a while that might not happen and if you have a capsular contracture, then a capsulotomy might solve the problem.
