Thank you for your question. Fat graing is a good option to make small changes especially around the cleavage area. It is not uncommon for implants, and breasts for that matter to fall slightly sideways when laying down. One way to treat this is by tightening the capsule either with stitches or with an energy device. However due to the large size of your implants it is likely that they will continue to slide slightly sideways overtime. Good luck.

The medial dent may be treated by releasing underneath (from the inside) or with a fat transfer. Large breasts commonly fall to the lateral chest wall in the supine position. Suturing of the lateral capsule (from the inside) or using a mesh to bolster the area would help this issue.
