Good Day. Many of my patients have the same concern. Perioral beauty centers on the following: 1. straight, white teeth 2. Optimal dental show- this optimal appearance is 2/3 of the height of the maxillary central incisor tooth when smiling and 0-3 mm at rest. 3. minimal perioral wrinkles, especially of the upper lip area 4. Full upper and lower lips How to achieve optimal dental show (not having gum show or a "gummy smile"): a. volume replacement of the upper lip. This can be achieved with a HLA filler to the border and bulk of the lip, using a soft silastic implant, or using your own tissue, either fat, scar, tissue from a facelift (SMAS) or the tissue below your scalp (pericranium). b. If the upper lip volume is optimized and the patient does not desire any more upper lip volume, then about 8 units of BOTOX (20 units of Disport) can relax the muscle of the upper lip (orbiculares oris) to hide additional tooth height. Rarely is any other treatment required to treat the gummy smile or excess dental show. Lip aesthetics use the following factors to determine lip size with an implant or fillers: upper and lower lip size ratio philtrum height-upper lip height proportions degree of dental show presence of upper lip wrinkles patients desired lip size The most influential factor in choosing the lip implant size was to optimize the lip size ratio of the upper and lower lip. One ratio was derived from the ancient Egyptian golden number (1.618) of facial aesthetics. Using this golden ratio, the upper lip height is a unit of 1 and the lower lip height a unit of 1.618 In summary, upper lip volume replacement with a HLA filler or silastic implant will improve and may resolve excess dental show known as a "gummy smile". Additional improvement may be attained adding an injection of BOTOX or Disport to the muscle above the upper lip (pre maxilla). I hope your final look is fabulous.
