I have a personal mantra in medicine: Make the correct diagnosis and then apply the correct treatment.I'm convinced many times when poor outcomes are achieved in cosmetic surgery, it's because the diagnosis was missed rather than the treatment not done correctly.Your chin projection is good, but your jaw profile is inadequate for the idea look.You may tolerate a small chin implant.You have mentolabial angle (this is the angle between your chin and lip) which will not tolerate anything but a small chin implant. I want you to look carefully at the patient Dr. Portuese has presented (Dr. Portuese has a superb reputation, and this has nothing to do with his talent or suggestion. I am merely using the patient he showed as a demonstration). Notice how the patient has a chin that sits significantly behind his lower lip. Notice how the shape of the chin is fairly square and does not stick forward. In comparison to your profile photo, your chin also sits quite a bit behind your lower lip, but the chin shows a forward projection. Dr Portuese's patient has a very different mentolabial angle. In his case, he was a perfect candidate for a fairly aggressive chin augmentation with an implant. You are not. Comparing your picture to the patient Dr. Portuese presents is a little bit like an apples and oranges comparison.Should you have anything but a small chin implant placed. you will develop something called a witch's chin. This is because you have a different mentolabial angle.So what to do?To get this done correctly requires an accurate and careful facial assessment.The fact that you were getting different opinions tells you that there are pitfalls to be had here, and you need to be very careful. Perhaps the correct procedure would be a mandibular advancement.This is quite different than a sliding genioplasty or chin augmentation with an implant.Unfortunately, that is a fairly invasive procedure that requires careful dental assessment and is generally done by oral surgeons or craniofacial surgeons.Without getting too technical, I want you to understand the limitations of chin augmentation while realizing your jaw projection is less than ideal.I don't believe any skin treatments or facial fat volume changes will help you. If you are serious about having intervention, I suggest you at least consult with one or more craniofacial-trained plastic surgeons or oral surgeons who perform aesthetic mandibular surgery.In the meantime, do some research on chin augmentation (with witch's chin) and mandibular advancements (avoiding extenuating the witch's chin).Some patients are slam-dunks for chin implants.
