30s, severe asymmetry after 2 pregnancies & breastfeeding, what would be the best surgical procedure?

I would recommend performing bilateral implant insertion and bilateral breast lists with internal bra for support. Implant selection is one of the most important aspects of breast augmentation/augmentation-mastopexy. To improve symmetry, you can consider either reducing the size of the larger breast to match the smaller with a breast reduction, or increasing the size of the smaller breast to match the larger with breast augmentation. A breast lift can be performed to improve perkiness at the same time, if desired.

Hi. You could get away with breast lift on one side and reduction on the other. This would be good if you don’t want implants, but with implants, I think you could get better symmetry. Your photos show a combination of postpartum sagging, significant asymmetry, and a variation of the tuberous breast deformity. In the smaller, right breast, there is deficient skin between the areola and the inframammary fold as well as an overall smaller size and diameter. The left breast is fuller, wider, and more saggy. You would benefit from a combination of surgical techniques all performed at one time, possibly including placement of at least an implant in the right breast or in both (although of different sizes), correction of the sagging and stretched areolae with skin tailoring, and rearrangement of the actual breast tissue to create a more normal and symmetrical appearance. You should expect a very nice appearance. Some of this depends on just what size you would like to be, and which breast has a more acceptable size for you, once the sagging is corrected and reshaping is complete.

The best solution is bilateral breast lift , left breast reduction and bilateral breast implants of different size. Your aerola are large and you right breast has dropped some. With that combination you shoukd be able to get good symmetry although not complete of course. I don’t think you can good symmetry without implants but the size is flexible depending on your desires.

Thank you for your question. You are an excellent candidate for a breast augmentation with mastopexy as well as a reductive augmentation of your left breast. I wrote the original paper on reductive augmentation in 2017. The idea behind it is to remove the extra breast tissue of your left breast so I can place the same size and style implant in both breasts.
