Im happy with Facelift results but the neck has just collapsed - what does revision require? (Photos)

Question

I had a mini lift at 59( that is what my surgeon called it ) and subsequently i had a deep plane face and neck lift at 64. My neck has NOT held Up and im super disappointed. What does a revision entail? Especially in terms of new sutured areas? I did have incision under chin.

Unfortunately it is more of a skin laxity and aging issue than a neck lift. No matter how hard you pull it will loosen. The best approach is to tighten the skin with lasers, microneedling etc. Sofwave is good for the tightening and Resurfx or Fraxel on the skin surface wrinkles.

It is very difficult to provide good, long lasting result in person that seem to have heavy, wrinkled, non-elastic skin like your neck skin. While your face result seems to be good, neck, in particular lower, looks wrinkled and heavy because of skin. Different skin treatments from Retin A, laser, chemical peel, radiofrequency, can to a certain degree improve skin texture and tone. A surgical correction by means of an extensive posterior neck lift, using incisions underneath the chin and behind ears is needed to correct current result. These two treatments need to be coordinated. Good luck.

Long lasting neck lift results with smooth skin surface, youthful skin quality, trim contours, and absence of neck bands demands a high level of expertise and a comprehensive approach by the operating surgeon and maintenance over time by patients.  Daily tretinoin on the neck and upper chest adds collagen, thickens epidermis, and resolves sun damage.  During neck lift, wide undermining and mobilization of skin and other technical elements facilitate skin advancement sufficiently for longer lasting smoothness.  Early smoothness after surgery without complete mobilization usually represents swelling.  Many months later after the swelling has resolved, the wrinkles return.
