Hi you didn’t mention how old you are and if you need fat anywhere else in the face.. paranasal area is a great place for fat transfer but usually requires at least two sessions for permanent volume restoration. If this area is your only concern, Radiesse filler can be initially placed to give you an idea and may be substantially more economical than fat injection, and can last up to a year..... my other favorite option to this area is your own cartilage implants harvested from the ears... We try to provide a delicate balance of fat transfer to multiple areas to "re-volumize" the loss as we age. It has been studied extensively where volume has changed with aging. The goal is to bring harmony to replace the deep fat pockets that have "deflated" over time and often it's combined with a facelift to re-suspend the descending tissues. If a facelift is not needed, we instead can use fat taken from either the stomach, flank, or outer thigh regions to provide the necessary volume to create a more youthful facial appearance. I recommend you seek a board certified plastic and reconstructive surgeon experienced with fat transfer to assist you in your goals. Paranasal augmentation can certainly be attempted first using your own fat. Whether it will survive sufficiently or not can not be precisely predicted before surgery. If not ou can then consider paranasal midface implant augmentation whose outcome is assured.

It seems at this point, it is more likely to be the fat causing this problem. To my knowledge the only way to fix this problem is to extract some of the fat through small incisions on the underside of the lip. Filler most likely would be resolved by now.
