Question

Revision surgery on left breast due to capsular contracture will I need an internal bra/mesh or can I benefit from just a lift? After my capsular contracture revision, the capsule was removed, and I was left with a breast lacking upper pole fullness. The breast over the last few months has descended into a more sagging position and is uncomfortable when I am not wearing a bra during showers and causes slight sharp pains . My question is how much of an extensive revision will I need ? I’ve spent so much money already on my revision due to my capsular contractor that I’m scared this will cost a lot of pocket and ultimately having to pay for another big revision. I do not have any firmness or swelling in the revised breast . The only issue I’m having right now is the sagging of the breast. Can this cause me to get another capsular contracture? Please tell me what are my options. And can this revision be done in office without having to spend a lot of money on a revision again .

Your breasts are low on the chest wall and ptotic. The skin of your breasts is touching the skin of your chest wall. Unfortunately, the weight of the implants has stretched out the pockets and your breasts are falling inferiorly and laterally. Also, your breasts are wider than your chest wall, which leads to mechanical instability. Your breasts will fall more to the side in the future. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

There will always be right to left differences before and after any surgery, especially with capsules.

Unfortunately, capsule contracture can squeeze the breast making it look fuller on top compared to the other side. When the capsule contracture is removed, the breast is softer and the implant appears less full on top.

Careful consideration to any revision as it risks return of capsule contracture. With mesh, it will be much lower but still a risk and mesh is quoted expensive. Larger implants for more upper fullness can be risky also.

Further in depth discussion with your/a plastic surgeon is recommended to determine whether a revision or no revision is best.

Based on the nicely, post photos, still best to do a virtual consultation for better explanation of options to be discussed and fee quote. Based on the photos, I would do an internal bra with a donut lift or L-shaped master taxi followed by which liposuction and fat transferred to proposal of breast. First. I am recently retired so I have no skin in the game here.

Your best long term results will be to remove your implants if you hate your breasts now.  Recurrent contraction is likely. If they are generally ok then delay.

When the time comes remove the implants and get a full lift with fat grafting to the upper pole.

A delayed reimport is an option but at this point don’t chase a problem that is likely to recur.
