Hi! I had a consultation and he recommended a 2 stage breast lift w/ implants (mentor). I'm 42, 1 pregnancy 2 years ago and breast fed for 3 weeks and open to getting pregnant again. The doctor said that getting pregnant shouldn't affect the lift and implants. I do have PCOS and did IVF for 5 years (IVF on pause) and my breasts did change very little. How do I convey to my doctor that I would like to stay the same size after the implants? Does a lift remove a lot of tissue? Thanks

Yes, it is possible to stay close to the same cup size after a breast lift with implants, but it requires very clear planning. Cup size is not exact, so it is better to explain your goal as: “I want my breasts lifted and reshaped, but I do not want to be noticeably larger.”
A breast lift usually removes mostly extra skin, not a large amount of breast tissue. However, the breasts can look slightly smaller after a lift because the tissue is tighter, higher, and more compact. If tissue is intentionally removed, then it becomes more of a reduction-lift.
Implants are added when more upper-pole fullness, roundness, or volume is desired. If you want to stay the same size, your surgeon may choose a small implant or may recommend lift alone/auto-augmentation depending on how much natural tissue you have.
Pregnancy can absolutely affect the result. Even if your breasts changed only a little with your last pregnancy, another pregnancy may still stretch the skin, change volume, or affect implant/lift position. If pregnancy is likely soon, waiting may be more efficient.
My advice would be to tell your surgeon directly: “My priority is shape and lift, not size increase. I want to remain close to my current cup size.” Ask whether a lift alone, auto-augmentation, or a very small implant would best achieve that. Also ask how future pregnancy could affect the result and whether staging is necessary in your case.

The breast lift technique I recommend is the Bellesoma. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in long term stability. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers or small implants can be placed if additional volume is desired.

A breast lift removes excess skin, reshapes the breast and raises the nipple position.  Often times a small implant can be used to help enhance the shape of the breasts to compliment the lift.  An implant doesn't have to equate to a much larger look.  A small enhancement can still give you a beautiful, natural result.
