Hello I had 2 lifts/reductions same surgeon in the last 7 years. Now I am looking to improve them again as I really don’t like the same and the nipples. Ideally want a round C cup with mode Centered nipples. How can I achieve that? Where should the scars be ? Can a melon slice scar achieve it?

From the photos you provided, your nipples are actually well centered on the breast mounds.  Moving them any more medial would not be optimal and would actually make them look less natural.

The only changes I would suggest would be to improve the scar appearance a bit through either surgical excision or laser resurfacing.  Otherwise, you have a nice result.

After two previous lifts or reductions, achieving a round C cup with more centered nipples may be possible, but it is a revision case and needs careful planning. The existing scars, nipple blood supply, tissue thickness, and breast shape all matter.
A “melon slice” or crescent-type scar is usually limited. It may move the nipple slightly, but it does not give strong breast reshaping or reliable centering when there is significant asymmetry, loose skin, or lower pole shape issues.
For more control, a vertical or anchor-pattern revision is often more predictable. These approaches allow the surgeon to reshape the lower breast, adjust nipple position, reduce areola size if needed, and improve symmetry more effectively than a small scar-only technique.
If you want a round C cup, you may also need volume planning. Depending on how much tissue you have, that could mean reshaping your own tissue, fat grafting, or a small implant. However, adding an implant after multiple lifts should be considered carefully because it can add weight and increase future sagging or revision risk.
My advice would be to see a revision breast specialist and bring your prior operative reports if possible. Ask what scar pattern gives the safest nipple blood supply and best control of shape. In most revision cases, the scar should be chosen based on the correction needed, not simply the shortest scar.
