is it possible to ask for a more conservative lift rather than a typical large lift even with severe ptosis to limit the risk of nipple necrosis? is it okay to tell my doctor i’m okay with my nipples being lower with the implants even with a lift? just a preference.

The lift technique I recommend is the Bellesoma. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers or small implants can be placed if additional volume is desired.

Yes—you should tell your surgeon that you prefer a more conservative nipple position and would accept some remaining droop. The planned nipple height is partly an aesthetic choice. However, simply lifting the nipple less does not automatically eliminate the risk of nipple loss. Safety depends more on preserving an adequate blood-supply pedicle, limiting tension, avoiding an implant that is too large for the tissues, and accounting for smoking or nicotine, diabetes, prior breast surgery, and the degree of ptosis.

With severe ptosis, an intentionally limited lift may leave the breast tissue and nipple sitting low on the implant or may produce recurrent sagging. Depending on your anatomy, safer options can include a smaller implant, a staged lift and augmentation, or a different pedicle design. Each has tradeoffs involving shape, scars, sensation, and breastfeeding potential.

Your preference is completely reasonable, but the safest design can only be chosen after measurements and an examination. I would ask the surgeon to show the proposed nipple level and explain how the pedicle, implant size, and staging decision affect both blood supply and the expected shape.

Hi Gorgeous! Absolutely let your surgeon know if you prefer a more conservative result and are comfortable with your nipples sitting slightly lower. Your aesthetic goals are an important part of planning your surgery!
However, performing a smaller lift does not necessarily reduce the risk of nipple necrosis. Nipple blood supply depends on many factors, including your anatomy, the technique used, the amount of tissue being moved, implant size, and tension on the breast tissues. A qualified board-certified or board-eligible plastic surgeon should evaluate these factors and choose a technique that safely preserves the nipple blood supply.
If nipple viability is a significant concern in your particular case, I would be more inclined to discuss staging the breast lift and augmentation rather than simply performing a less complete lift. Staging allows the breast to heal before adding the additional volume and tension of an implant and can be a very good option in higher-risk situations.
It is completely reasonable to prefer a more natural, slightly lower nipple position, but ideally that decision should be based on the look you want rather than as the primary method of reducing surgical risk.

In my experience, the vertical breast lift techniques with or without an ipkant have a VERY low rate of nipple loss in non-smokers.  If your nipples are exceedingly low, then a staged procedure might make sense.  If you do not desire a nipple lifted to the standard position, then of course your surgeon can accomodate that.

Yes, this is something you can absolutely ask for, and it is a reasonable preference to state openly. Limiting how far the nipple is moved upward does reduce the tension and the distance the blood supply has to travel, so in some patients a more conservative lift is genuinely the safer choice, and accepting a slightly lower nipple position is a legitimate trade off. I would also add that adding an implant does not automatically mean higher risk, since much depends on the implant size, the pedicle design and your tissue quality. Whether a limited lift will give you a result you are happy with can only be determined by physical examination.

With severe ptosis, moving the nipple a long distance can increase concern about blood supply, especially when combined with implants. The risk depends on your measurements, breast size, skin quality, smoking/nicotine use, prior surgery, medical history, and the lift technique used.
A more conservative lift may reduce tension and may be safer for the nipple-areola complex, but it also means accepting a less “perfectly lifted” or less high-positioned result. This can be a valid trade-off if it matches your goals.
Implants can add upper fullness, but they do not replace a lift. If the nipple is left too low relative to the implant, there is a risk of a low-nipple or “waterfall” appearance, where breast tissue hangs off the implant. So the plan needs balance: safe nipple movement, good implant position, and realistic breast shape.
My advice would be to clearly tell your surgeon: “I prefer a safer, more conservative lift and I am okay with a slightly lower nipple position.” Then ask whether that would still create an acceptable shape, whether staging the lift and implants would reduce risk, and what technique best protects nipple blood supply in your case.

Being conservative with the breast lift is an option, but it’s probably better to do these as split procedures. If you are that concerned about severe complications likenipple necrosis.
Abreast augmentation with a lift, especially in cases with significant breast droopiness has a fairly high revision rate as it is. A paragraph doing these a separate procedures can increase quality outcomes and minimize risk.
In the end, provider selection is probably the most important variable.
A recommend you have several in person consultations with plastic surgeons in your community.
As providers to open up their portfolio and show as many before and after pictures of previous patient who had similar breast characteristics to your own.
Seeing one or two outcomes is insufficient to get a clear understanding of how much experience the provider has or whataverage results tend to look like.
You should ask providers how many cases of nipple necrosis they’ve had in their career and how each one managed and with the worst case scenario was.
Any surgeon who does a lot of breast surgery is going to have some complications.
 The most experienced surgeons will probably also have affair number of complications because of the number of cases they’ve done.
It’s still gonna be the experienced providers that are going to do the best work
Different plastic surgeons have different approaches. Being conservative with implant size can also help reduce thechance of having problems and need revision surgery.
