If there is some persistent residual glandular tissue, it can may be removed at that time.It is difficult to offer any real advice without photos. Yes, on occasion the results of gynecomastia surgery are not acceptable to a patient.l Realize that revision or secondary surgery is always playing catch-up -- the best chance for a great result is the first procedure.Revision may require additional fine liposuction of the chest and/or gland excision under the nipples. Sometimes, too much tissue has been removed which leaves a crater deformity. These are challenging cases but oftentimes the crater can be reconstructed with adjacent tissues. Sometimes there are poor scars -- and often they can be improved through scar revision. And sometimes there is just some scar tissue (often under the nipples) which may require only a cortisone injection.You are best advised to seek the opinion of a gynecomastia expert in your area -- or consider traveling for such expertise. One of the problems with gynecomastia treatment is the removal of too much tissue or not enough tissue. The "art" of the procedure means that you often get this balance just right. This is a happy patient. Leave too much tissue or don't remove enough and often the patient isn't happy and if there is too much tissue more needs to be removed.When do you consider removing this tissue excess? It depends, but often the swelling isn't that significant and if you can appreciate the tissue it simply needs removal with additional contouring of the chest as needed.
