Without a full series of photographs or an in-person examination it is difficult to offer definite advice.  It is possible to have gynecomastia or pseudogynecomastia present in only one side of the chest that can result in a puffy appearance to the nipple.  I would recommend seeing an ASPS board certified plastic surgeon in consultation to discuss your treatment options.  

Without photos, it is really hard to say what is going on.  Puffiness can be the result of a number of factors but is generally caused by increased breast gland beneath the nipple.  This glandular tissue can be removed surgically with very little discomfort and downtime. Best to consult with a Board Certified Plastic Surgeon to discuss your options.I hope that helps!

I would suggest that you schedule a consultation with a board certified plastic surgeon to be examined in person and find out exactly what is causing your issue. Gynecomastia is easily fixed with surgery.

Unilateral gynecomastia is pretty common and is more noticeable. The breast size may increases over the time.
This is an aesthetic condition,so you may elect surgery if it bothers you.
