I'm a 21 year old male very skinny, and one of my nipples is puffy and the other is flat. When I was younger there was a little lump in it but that is gone, other nipple had the same problem but is now flat again, what about this one?

It is possible to have gynecomastia or pseudogynecomastia present in only one side of the chest that can result in a puffy appearance to the nipple. I would recommend seeing an ASPS board certified plastic surgeon in consultation to discuss your treatment options.

However, puffiness is often the result of increased glandular tissue. I would suggest that you schedule a consultation with a board certified plastic surgeon to be examined in person and find out exactly what is causing your issue. Gynecomastia is easily fixed with surgery.
