Thank you for your question. It all really depends, and a detailed conversation with your surgeon is important. Typically osteotomies are needed to narrow the nasal bones. If this is your first time having surgery on your nose, you may not require rib grafting to achieve the result you're aiming for, which would be a bonus. Based on your photos, the tip doesn't appear to be all that low. It could be rotated a little, but I'd be careful to as to avoid over rotation. Hello! Thank you for your question. As far as I can see from your photos you nose is wide, tip of your nose is very deprojected. For gain more projected, narrow and elegant looking I recommend you rhinoplasty with alar base plasty. For the rib cartilage your surgeon will decide it in the surgery after see inside your nose. It is very hard to decide whether you need dorsum augmentation at all? No doubt that tip improvements and support by means of cartilage grafts are needed, as well as narrowing of the nose by means of osteotomies. Second opinion, in person consultation, with a board certified plastic surgeon with good reputation in rhinoplasty-ethnic rhinoplasty, is recommended. Good luck

When we need to increase the relative strength of the top of your nose, adding structure is key. A rib graft can be taken from your own body (in a small crease under the breast) or obtained as a sterilized donor graft. These tissues can be used to fill in the top of the nose and increase support. That will, along with moving the nasal bones, also relatively narrow the nose for a more defined and sculpted appearance.

Hi there! Thanks for the questions. A rib for dorsal augmentation can perform very well. It will raise your bridge on profile view, though the degree to which it does can be adjusted. If the graft is narrowed, it will make the frontal view appear more slender. Osteotomies are an option if the profile is set to where you want it. The midvault could be narrowed as well avoiding the need for rib (only use septum instead if enough is present). Tip refinement would involve dome sutures, grafting, and likely projection of a 2-3 mm to accomplish narrowing. Seek consultation with a facial plastic surgeon who performs rhinoplasty often. Every nose is different and unique to that individual. If the appearance of your nose concerns you enough to do something about it, it can be improved with a natural artistically performed rhinoplasty. What would be the best option and exact procedure would depend on an in person examination (not just an online consultation) and discussion about desired outcome. Excellent results can be achieved with either a closed or open technique.

Keep in mind that following advice from a surgeon on this or any other website who proposes to tell you exactly what to do and how technically to do it based on limited photos without examining you, physically feeling your tissue, assessing your desired outcome, taking a full medical history and discussing the pros and cons of the operative procedure would not be in your best interest. Natural appearing results need to be individualized and what is appropriate for one patient is not necessarily the best for someone else. I would suggest that you find a surgeon with extensive rhinoplasty experience certified by the American Board of Plastic Surgery and one who is ideally a member of The American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. It's very important to understand that the nose is a three-dimensional structure, and adding a piece of rib to the bridge line will raise the height of your bridge from the side profile which would look odd. Your side profile looks great, it's very important not to change it. You do not have a droopy tip. A closed rhinoplasty can accomplish refinement of the nasal tip cartilages to narrow them from the frontal profile, and osteotomies placed in the nasal bones to narrow the upper two thirds of your nose which is what you're trying to accomplish. These are called medial and lateral osteotomy of the nasal bones. Choose your surgeon wisely based on extensive experience producing natural results.
