Hi, I've been considering getting a nose job done for a while now but am unsure if I am a good canidate for just an alarplasty. I feel like my nose is quite short but my main concern is the width and uneveness of my nostrils. Would an alarplasty address these issues or would a full nose revision be a better route to take? I feel like the second option could help decrease the overall width of my nose but im not sure if that would fix the uneveness of my nostrils.

Answer: Alar excision An alar excision can be done for wide nostrils if that’s what bothers you but you have to be careful if you have a short columella then you can not remove to much alar or you will affect the size of nasal aperture

Answer: Do I need an alar plasty or a full rhinoplasty It depends upon what's bothering you. If your wide nostrils are bothering you, just perform an alar-plasty which narrows them. They're asymmetrical now, and they will still be asymmetrical after the surgery, just narrowed. An alar plasty can be performed under local anesthesia. If you want to narrow your bridge line, that's a full rhinoplasty and that must be done under general anesthesia, since it's bone work. Your side profile does not need any improvement. You could always do the alar plasty first, and after a year or so if you're still unhappy with your nose, you can always undergo a full rhinoplasty.

We recommend that you undergo treatment to the specific areas of your concern. Narrowing the nose will not correct the notch (the nose can be narrowed if this is of concern to you). A tip-plasty will increase tip definition. Addition of a small cartilage graft will fill out the depression and excision of the alar bases will narrow the nostrils and correct nostril flare.
