In early 2019 I had a closed rhinoplasty to remove a dorsal hump. The hump was small enough that no osteotomies were performed. Now I fear I have an open roof and a slight pollybeak. I don't want my tip overly lifted (which is a fear I have with revision). Could I fix the bridge and pollybeak with filler or do I need a revision? Note: I've had a ton of steroid shots and even a 5-fu shot in my nose and the pollybeak has only gone down a tiny bit.
Answer: Filler and the Nose Hi there! You definitely can try filler in your nose to provide camouflage to the pollybeak and open roof. There are a number of different fillers which can work well such as Juvederm Ultra. You would likely have to raise your radix slightly and/or project your nose to reduce the appearance of the supratip fullness. A revision would likely get you more precise and final results. With filler, there is always a risk of creating scar tissue and damage to the skin. Hope this helps!

Answer: Revision rhinoplasty? Based on your photos, you are a good candidate for revision rhinoplasty. The osteotomies can be better completed to give you a better bridge. The tip can also be supported and projected better without rotating the tip up. If you don't want surgery, a filler can improve the shape from the side as well. Be sure to see a rhinoplasty expert for consultation.

Filler is a relatively low-risk way to help make smaller changes to the nose, which in this case may be appropriate. However, the result is not permanent. If this issue is a huge bother, then surgery may be the better option. However, keep in mind that surgery is more invasive and carries higher risks. If you're mild polybeak's due to thick skin, steroid shots with 5-FU will help. If you're Polly beak is due to excess cartilage, then a Revision rhinoplasty is required. Nasal bone Osteotomies are required to narrow your wide nasal bones and close the open roof. In addition a revision can accomplish reduction of the hanging columella that you currently have. Your a revision rhinoplasty can be performed with a closed technique to avoid the external scar across the hanging columella. All incisions are placed on the inside of the nose and no painful packing is required either
