Question

Hello, my right eye is lower than my left eye. I am thinkink about gettlng an eyelid surgery to fix that. What do you recommend should I do only one eye or both? Also what can be done for under eye puffiness? Thank you for your response. I am 24 years old.

Thank you for your questions and photos. 
It looks like you have a mild degree of ptosis and some asymmetry of your upper eyelids which could be improved with ptosis repair (often performed by oculoplastic surgeon or plastic surgeon that specializes in eyelids). You also have some fullness of the lower eyelids which is likely congenital given your age. This could also be improved with surgery (lower blepharoplasty with fat transposition or removal). However, my first recommendation would be to rule out thyroid eye disease (labs can be ordered by primary care) as this can also lead to issues like yours at a younger age than average. I think your best bet is to schedule a consultation with someone who specializes in eyelids/eyes to discuss all of these options and determine your best steps forward.

Your major underlying condition is small cheek bones, malar hypoplasia which is the reason for an early onset of lower eyelid issues. Correction of cheek deficiency with cheek implant or fat grafting and lower blepharoplasty would correct this issue. It is very likely that you have mild upper eyelid drooping, condition known as ptosis. This can be confirmed only on the basis of in person evaluation. All three of these problems can be addressed in the same surgery. Good luck.

One side gets more brow effort. But both need surgery at the same time. Most Oculoplastic surgeons you consult will offer you a posterior approach Müllerectomy. That surgery will not provide an adequate repair in my opinion. You have a very strong compensatory eyebrow elevation. In order to get the brows to relax sufficiently, the anterior levator disinsertion needs to be repaired. You also need an anchor blepharoplasty to the upper eyelashes. I would encourage you to also have a transconjunctival lower blepharoplasty to deal with the lower eyelid fat. There is no substitute to a detailed in person assessment.

Based on the photos, your right upper eyelid appears slightly lower than the left, which may suggest mild ptosis rather than excess skin alone. If this is confirmed during an in-person examination, a ptosis repair procedure could help improve symmetry and eyelid position.
Regarding the fullness beneath the eyes, there appears to be some prominence of the lower eyelid fat pads. In a younger patient, a transconjunctival lower blepharoplasty is often an excellent option, as it allows adjustment of the fat pads without leaving a visible external scar.
A thorough examination would be necessary to determine the exact cause of the eyelid asymmetry and to discuss the most appropriate treatment options for both the upper and lower eyelids.
