I’m 18 years old and female and was curious if it’s possible to make an eyelid crease less visible, I hate how mine makes my eye look sunken in and I wish my upper eyelid looked more flat, just curious if something like that is even possible

At 18 years old, a visible upper eyelid crease is usually a normal anatomical feature rather than a sign of aging or volume loss. A deeper or more defined crease can sometimes make the upper lid appear slightly hollow, but in young patients this is typically related to natural eyelid structure and how the levator muscle attaches to the skin. Surgically “removing” or flattening a natural crease is not a simple or commonly recommended procedure, especially if your eyelid function is normal and there is no true hollowing or ptosis. Upper blepharoplasty is designed to remove excess skin and would likely make the crease more defined, not less. In selected cases where true upper eyelid hollowing exists, subtle filler placement can soften a sunken appearance, but this is usually reserved for older patients with volume loss, not for healthy 18-year-olds. In most young individuals, makeup techniques, lash styling, and brow shaping can significantly change how prominent the crease appears without surgery. Any surgical attempt to permanently alter a natural crease at your age would carry unnecessary risk and unpredictability, so conservative, non-surgical approaches are strongly preferred.

In response to both your question and another response, lowering your eyelid crease is quite possible, a delicate office surgery under local anesthesia.  The upper eyelid crease ("supratarsal" or "orbito-palpebral" crease) represents the lower limit of orbital fat distribution across the upper eyelid.  Fat does not drop below the crease because skin and orbicularis muscle are adherent to the underlying tarsal plate all the way down to eyelashes and the lid margin.  To lower the crease, simply mark where you want the crease, remove skin between the existing crease and where you want the crease, usually only 2-4 millimeters, and re-suture the skin and orbicularis along the new lower crease.  Orbital fat will drop down to the level of the lowered crease assuming the orbital septum has been mobilized.

Changing a crease, and specifically lowering a crease is very challenging surgery. Fixing underlying eyelid ptosis (droop), which is difficult to see from your photo, can also help. Seek a surgeon that has special training in Asian eyelid surgery, as this is very commonly done in that patient population. See link below.
