Hello! I have had these deep folds under my eyes since a baby is it a tear trough if so can it be fixed with lower blepharoplasty? Are there any surgeries you would recommend? I am 20 and female and of south indian orgin

From the current photo, it is not possible to clearly assess whether there is a lack of projection in the cheekbones. A full face photo taken in good lighting would allow for a much more accurate evaluation.
Based on what can be seen at this stage, there does not appear to be significant fat protrusion or skin excess, which would justify lower blepharoplasty. The concern seems to be related to contour and skin tone rather than true excess tissue.
Improvement may be achieved by first addressing the darker appearance of the skin with treatments such as mesotherapy or skin quality focused procedures. After that, tear trough filler can be considered to refine the hollow if needed.
A more detailed assessment would help determine the most appropriate step, but surgery does not appear to be immediately necessary based on the current findings.
